W-4 Form


All new hires must provide the Payroll department with Forms W-4.  The amount of employees' federal income tax withholding, and, often, state income tax withholding, is determined by entries on this form.  Employees indicate their marital status and the number of withholding allowances on these forms.  Employees may also indicate that they're exempt from income tax withholding.  Worksheets are attached to assist employees in making these calculations.  W-4 forms remain in effect until employees update them.  Forms must be signed under penalties of perjury.  The IRS updates this form every year.  Continue W-4 Form

  1. Can an employee claim an exemption from income tax withholding if he/she gets a tax refund the year before?

  4. An employee has a habit of changing her W-4 form several times a quarter. It doesn't seem tied to any particular payment or done with an apparent intent to tax protest, but it is annoying, since we're always having to deal with her revised forms at the same time we're trying to get the payroll out. Just how many times a year can an employee refile a W-4?

  5. If an employee has a name change and provides the proper documentation of the change to Payroll, is she required by law to fill out a new W-4 form?
